This section contains the forms and policy documents required to complete registration for the Microbiology Services' web-based report & document delivery system which will enable you to receive laboratory reports from MS Colindale as PDF files.

What you need to do to register

STEP 1: Complete the registration form and return it to us at the address or fax below:

STEP 2: We will email you to confirm receipt of your registration form and agree the start date (please note we need 24 hours notice)

STEP 3: On the agreed start date, we will activate your Lab to receive web-based reports and your local administrator will receive an email with their user log-in details

Other users can either register individually (using the HPA Customer Code which their local administrator can confirm), or the local administrator can do it for them. Either way a user will not be able to access HPA-eLab until the local administrator has approved their application within the system.
