LHCAI/ARMRL Services request forms

There are three available request forms for LHCAI/ARMRL services.

H2 Healthcare pathogens (single isolate) form (PDF, 360 KB): Single isolates for standalone investigations
H1 Healthcare pathogens (multiple isolates) form (PDF, 370 KB): Submission of multiple isolates of the sample species which are part of a batch investigation and epidemiologically linked.
H1 - Healthcare pathogens cont. page (PDF, 391 KB): H1 (cont) is a continuation page for the H1 form and should ONLY be used with the H1 form to submitt more than 4 samples for the same service.

There are two available request forms for FSML services.

L6 Clinical Specimens/Cultures Form (PDF, 327 KB): Clinical specimens/Cultures isolated from clinical specimens
L7 Food Samples/Cultures Form (PDF, 326 KB): Food/Cultures isolated from food

There are two available request forms for enteric virus testing.

There are two available request forms for the Mycology Reference Laboratory. Please select the one most appropriate to you based on the notes below:

Y1 Mycology identification/susceptibility testing form (PDF, 312 KB): Referring isolates of yeast or mould for identification and/or susceptibility testing
Y2 Diagnostic mycology testing form (PDF, 320 KB): Referring clinical samples for diagnostic mycology testing
