Surveillance Forms

In the first instance, completed Surveillance Forms should be sent to:

A copy of the completed surveillance form will then be forwarded to the relevant section at CfI, Colindale by HPA NW Regional Office.

• Listeria surveillance – questionnaires (both exposure questionnaire for interviewing cases and clinical questionnaire for completion by microbiologists)
• LGV (Lymphogranuloma venereum)
• Meningococcal Disease
• Tetanus surveillance form
