Surveillance Scheme

Objectives of the Scheme

  • To detect clusters or outbreaks of legionella infection in England and Wales or abroad.
  • To identify sources of infection so that control measures can be applied to prevent further cases.
  • To disseminate legionella surveillance information to al those who need to know.

National Surveillance Scheme Reporting Form

All confirmed cases of Legionellosis should be reported to the National Surveillance Scheme for Legionnaires' disease using the National surveillance form below.

nb: if you would prefer an electronic version of the National surveillance form please e-mail legionella@hpa.org.uk with your request and an appropriate form will be sent to you.  Alternatively, please report any cases you have by printing the above form, completing all the details and faxing it to the Centre for Infections on 020 8200 7868.

Further information can be requested from the Legionella Epidemiology Section, at the Health Protection Agency, Centre for Infections, by completing and submitting the information request form below.

Investigation Reporting Forms A and B

If two or more cases of Legionnaires’ disease (a cluster) are identified as being associated with any accommodation site within a two year period, a risk assessment needs to be caaried out and the findings reported to the European surveillance scheme (ELDSNet).  

A preliminary report using the 'investigation reporting form A' needs to be completed within two weeks of a cluster being identified.
