Payment by Credit Card
Please read the following information before completing the 
Publications Order Form (PDF, 34 KB) or the RPD Credit Card Authorisation Form (PDF, 12 KB) for Training Courses.
These forms should be used to authorise payment by credit card for goods and services from the HPA Radiation Protection Division in the following categories.
- Publications, including documents, reports, leaflets and software.
- Training courses.
The relevant form should be printed off, the details completed and sent to the appropriate address below.
Only the types of card specified can be accepted.
Please ensure that a despatch address is provided with your order; otherwise we will have to use the cardholder's address.
Please note that orders can only be processed with credit card payment if all the information required has been provided and if the total payment authorised is quoted in pounds sterling.
