Since 2007 the largest known outbreak of Q fever has been ongoing in the Netherlands, with more than 3500 human cases reported to date. Whilst this outbreak appears to be related to factors unique to the Netherlands, clinicians in the UK should be alert to imported cases in travellers returning from affected areas. Recent cases have also been reported in personnel serving in Afghanistan. Q fever is endemic in the UK with approximately 50 – 100 acute cases per year. Therefore a history of travel to other countries is not necessary to make the diagnosis.
